New Passport Application Appointments
Each person applying for a passport must have their own appointment.
Need to apply for a new U.S. passport? The Norwalk Public Library is an official passport acceptance facility and offers appointments for new passport applications only (renewals are not processed at this location).
Applicants must apply in person and bring the following:
- Completed DS-11 passport application form (do not sign until instructed)
- Original birth certificate with raised seal or naturalization certificate
- Valid driver’s license or government-issued photo ID (plus a photocopy of front and back)
- One passport photo (available at CVS or Walgreens)
- Payment by check or money order only
Two separate payments are required:
- U.S. Department of State for the passport fee
Norwalk Public Library for the $35 acceptance fee
**Expedited service is available for an additional fee.
Children under age 16 must apply with both parents/guardians present, or provide a notarized consent form from the absent parent.
Applicants who do not speak English must bring their own interpreter; library staff cannot provide translation services.
Appointments are required. Please review passport requirements carefully before your appointment.
